Role of the Job
The Care Community Support Specialist (CCSS) is responsible for recovering no contacts, disenrollment, completing Patient-Reported Outcome Measures (PROMS), graduations, and return to program. The CCSS collects member data on reasons for no contact or disenrollment and directly schedules the next coaching session in the assigned coach’s calendar.
