About the Role
Croydon Health Services NHS Trust is expanding its Remote Monitoring and Virtual Ward service to improve patient care across South West London.
As a Remote Monitoring Hub Support Worker, you will remotely monitor patients with acute and chronic health conditions, helping prevent unnecessary hospital admissions while supporting safe discharge from hospital. Working alongside multidisciplinary teams, you will identify patients whose conditions are deteriorating and ensure timely escalation to clinical teams.
Key Responsibilities
- Monitor patients remotely through the Virtual Ward service.
- Support patients with acute and chronic medical conditions.
- Escalate deteriorating patients promptly to the appropriate clinical team.
- Work closely with community teams and multidisciplinary professionals.
- Assist with safe hospital discharge planning.
- Help reduce avoidable hospital admissions.
- Maintain accurate patient records.
- Deliver safe, efficient, and patient-centred care.
- Support the ongoing development of the Remote Monitoring Hub.
Essential Requirements
- Registered Nursing Associate on the NMC Register.
OR
- Overseas-qualified Medical Doctor awaiting or working towards GMC registration.
- Level 5 Foundation Degree in Nursing Associate Practice.
- Experience working in an acute NHS environment.
- Experience delivering patient-facing care.
- Experience working as part of a healthcare team.
- Good communication, organisational, and assessment skills.
- Ability to prioritise workload and work to deadlines.
Desirable Qualifications
- Care Certificate.
- GCSE English and another subject (or equivalent).
- Experience working within a multidisciplinary team.
