About the Role
The Stroke Association is seeking an experienced and compassionate Support Coordinator to join its Stroke Recovery Service.
You will support stroke survivors and their families from the point of hospital discharge and throughout their recovery in the community.
Working closely with stroke survivors, carers, NHS colleagues and other community organisations, you will provide personalised information, advice and support while helping individuals identify goals and maximise their recovery.
A person-centred approach and strong assessment skills will be important in helping people affected by stroke improve their wellbeing, communication and independence.
Key Responsibilities
- Support new stroke survivors and their carers following hospital discharge and into the community.
- Provide personalised information, advice and practical support.
- Help stroke survivors identify and work towards individual goals.
- Support people to make informed lifestyle changes that may help reduce the risk of further strokes.
- Build positive relationships with stroke survivors, families and carers.
- Work collaboratively with NHS colleagues and community organisations.
- Maintain accurate records and effectively manage a caseload.
- Provide support in people’s homes and community settings across the geographical locality.
- Contribute to the delivery of an effective and person-centred stroke recovery service.
Requirements / Person Specification
The successful candidate should have:
- Experience or a background in a caring profession.
- Ideally, experience supporting people with disabilities or other support needs.
- Excellent interpersonal and communication skills.
- Strong IT skills and the ability to maintain accurate records.
- Good organisational and caseload-management skills.
- A flexible approach to work.
- An ability to work collaboratively with NHS and community partners.
- An understanding of and commitment to the values of the Stroke Association.

Working Pattern
This is a part-time position for 28 hours per week.
The role is home-based, but it is not fully remote because extensive travel is required across the locality to meet stroke survivors in their homes and community settings. Travel may also be required for team and other work-related meetings.
