About the Homeless Health GP Role
North West London has a large homeless population, with people experiencing homelessness facing significant inequalities in health outcomes, access to healthcare and overall experience of health and care services.
People experiencing homelessness can have multiple and long-term health needs that are often under-treated. They may also experience difficulties accessing appropriate healthcare and face significant challenges when being discharged from hospital.
The specialist Homeless Health team works alongside hospital wards to identify people experiencing homelessness, establish therapeutic relationships, support multidisciplinary teams and help ensure vulnerable patients receive appropriate care.
The team also supports patients during and following discharge, helping them connect with the healthcare, housing and support services they require.
As the Homeless Health GP, you will provide clinical expertise and leadership to help improve the quality of care received by homeless patients in hospital and support better outcomes following discharge.
Key Responsibilities
The successful candidate will work as part of the Homeless Health team to improve health and social outcomes for people experiencing homelessness.

- Clinically lead, support and promote the specialist Homelessness team within the hospital.
- Provide high-quality clinical care to patients under the Homeless Health Service.
