About the Role
London North West University Healthcare NHS Trust is recruiting a Senior Health Care Support Worker – Intestinal Rehabilitation to join its multidisciplinary Intestinal Failure and Rehabilitation team.
Working under the guidance and supervision of the specialist nutrition nursing team, the successful candidate will contribute to the delivery of safe and effective care for patients with intestinal failure.
The role combines patient support, service coordination, administration and communication responsibilities within a specialist clinical environment.
Key Responsibilities
The successful candidate will be responsible for:
- Managing the Intestinal Failure Database and Registry and ensuring patient information is accurate and up to date.
- Supporting service reporting requirements through effective record keeping.
- Coordinating Nutrition Nurse activities and clinic schedules to support efficient service delivery.
- Creating and managing patient encounters for the Nurse-Led Rapid Access Clinic.
- Supporting patient pathways and ensuring relevant arrangements are completed promptly.
- Coordinating the timely provision of ancillary supplies and equipment for patients receiving IV therapies.
- Leading the administration and follow-up of the Nutrition Nurse Advice Line.
- Ensuring calls received through the advice line are accurately documented, appropriately actioned and followed through to resolution.
- Maintaining confidentiality and demonstrating appropriate accountability within the Senior Health Care Support Worker role.
- Communicating effectively with patients, healthcare professionals and members of the multidisciplinary team.
Requirements / Person Specification
Essential qualifications:
- Functional Skills Level 1 in Maths and English, assessed through BKSB, or equivalent GCSE qualifications.
- Completed Care Certificate.
Desirable qualifications:
- Level 2 or Level 3 Health Care qualification or apprenticeship.
- Completion of Band 3 Excellence in Care.
- Relevant competencies for existing substantive Band 2 Health Care Support Workers who have worked within the Trust for at least 15 months.
Experience and Knowledge
Applicants should have:
- Experience working in an acute hospital setting in a Band 2 Health Care Support Worker role or equivalent.
- Experience communicating with patients and healthcare professionals by telephone.
- Basic knowledge of IT systems and experience using computers.
- An understanding of the responsibilities, accountability and confidentiality expected within a senior health care support role.
- A basic understanding of the NHS and the roles of nurses, doctors, dietitians and pharmacists within an Intestinal Failure and Rehabilitation team.
- The ability to communicate clearly and concisely by telephone, face to face and in writing.
Desirable experience includes:
- Experience using NHS digital systems.
- Willingness to undertake training on local computer systems.
- Previous experience supporting patients with complex health needs.
- Evidence of continuing professional development.
- A good attendance record.
Salary & Benefits
The position is offered at NHS Band 3, with a salary of £30,630 to £32,346 per year, inclusive of the High Cost Area Supplement.
The post is permanent and full-time.
