About the Role
Join the Community Learning Disability Team, working alongside health professionals to support adults (18+) with diagnosed learning disabilities.
Key Responsibilities:
- Assess care and support needs in line with UK legislation
- Safeguard vulnerable adults and manage risk cases
- Conduct Mental Capacity Assessments and Best Interest decisions
- Support individuals lacking capacity, including Court of Protection processes
- Manage a caseload and maintain accurate records
- Handle Continuing Healthcare (CHC) funding applications
Requirements
- Registered with Social Work England
- Strong communication and teamwork skills
- Ability to manage workload independently
- Commitment to delivering person-centred care
