About the Role

Join the Community Learning Disability Team, working alongside health professionals to support adults (18+) with diagnosed learning disabilities.

Key Responsibilities:

  • Assess care and support needs in line with UK legislation
  • Safeguard vulnerable adults and manage risk cases
  • Conduct Mental Capacity Assessments and Best Interest decisions
  • Support individuals lacking capacity, including Court of Protection processes
  • Manage a caseload and maintain accurate records
  • Handle Continuing Healthcare (CHC) funding applications

Requirements

  • Registered with Social Work England
  • Strong communication and teamwork skills
  • Ability to manage workload independently
  • Commitment to delivering person-centred care
