About the Role
Somerset Council is seeking an experienced Advanced Practitioner Social Worker to provide professional leadership within its Adults Neighbourhoods Service. This role focuses on supporting adults to live independently within their communities through strengths-based practice, safeguarding, and integrated care.
The successful candidate will work closely with social care teams, health professionals, and community partners to support complex cases, promote independence, and improve outcomes for residents across Somerset.
Key Responsibilities
- Provide professional leadership within the neighbourhood team.
- Support and oversee complex adult social care cases.
- Promote independence and preventative approaches.
- Lead safeguarding practice and complex risk management.
- Provide reflective supervision to Social Workers and Adult Social Care Practitioners.
- Collaborate with health partners and Primary Care Networks (PCNs).
- Build relationships with community and voluntary organisations.
- Contribute to service development and improvement initiatives.
- Support workflow management and professional decision-making.
Requirements
- Qualified Social Worker.
- Current registration with Social Work England.
- Extensive post-qualification experience in Adult Social Care.
- Strong knowledge of the Care Act 2014, Mental Capacity Act 2005, and safeguarding legislation.
- Experience supporting practitioners and managing complex cases.
- Excellent communication, leadership, and partnership-working skills.

Visa Sponsorship Information
Somerset Council may offer Skilled Worker visa sponsorship to suitably qualified overseas social workers who are already living in the UK. Applicants must possess a recognised social work qualification and hold valid Social Work England registration.
