About the Role
The successful Social Worker will join the Hospital Assessment and Recovery Team (HART), supporting adults leaving hospital and helping them safely transition back into the community.
The role focuses on:
- Hospital discharge planning
- Adult social care assessments
- Safeguarding vulnerable adults
- Promoting independence
- Strength-based social work practice
Key Responsibilities
- Assess adults requiring social care support.
- Develop person-centred support plans.
- Work with multidisciplinary healthcare teams.
- Promote independent living.
- Conduct safeguarding enquiries under Section 42.
- Complete Mental Capacity Act (MCA) assessments.
- Prepare Human Rights assessments.
- Support homeless residents and those with NRPF.
- Manage complex family situations.
- Complete court-compliant mental capacity assessments.
- Participate in Best Interest meetings.
- Maintain accurate case records.
- Deliver legally compliant social work interventions.
Essential Requirements
