About the Role
The High Risk Young Adults Service supports young people aged 16–25 who are socially vulnerable or clinically high risk and living with long-term conditions.
The Social Worker will support young adults and their families as they transition from paediatric to adult healthcare services.
The role may involve supporting young people living with:
- Diabetes
- Heart disease
- Kidney disease
- Other long-term health conditions
- Mental health and social difficulties
As this is a new post, the successful candidate will have the opportunity to contribute to the development of the service.
Main Responsibilities
- Build effective relationships with multidisciplinary healthcare professionals.
- Act as a key contact for young people and their parents or carers.
- Provide person-centred social care support.
- Work with education and voluntary-sector organisations.
- Identify suitable non-clinical interventions.
- Develop relationships with relevant stakeholders and agencies.
- Attend staff meetings, supervision and training.
- Support young people through healthcare transitions.
- Contribute to service development.
- Help improve outcomes for young adults with long-term conditions.
Essential Qualifications
Applicants must have:
- A Diploma-level qualification in child development, youth work or education.
- A postgraduate qualification in Social Work.
Essential Experience
Applicants should have experience of:
- Working with children and young people in a person-centred manner.
- Working within health, social care or community settings.
- Maintaining confidentiality when handling sensitive personal information.
- Demonstrating impact and positive outcomes for service users.
Desirable Experience
- Experience working with young people with physical health conditions.
- Experience working with young people with mental health conditions.
Essential Knowledge
