About the Role

The successful candidate will work as part of a multidisciplinary Community Locality Team, providing specialist care and support to people with psychosis and complex mental health needs.

The role involves supporting patients and their carers, promoting recovery and wellbeing, managing clinical risks and working closely with other healthcare and partner organisations.

The successful candidate will also undertake regular monitoring of mental state, depot administration, comprehensive mental health assessments and risk assessments.

Key Responsibilities

The Key Worker/Care Coordinator will be responsible for:

• Providing holistic care to service users with severe and enduring mental health conditions.
• Supporting service users throughout their recovery pathway.
• Providing specialist advice to patients, carers and colleagues.
• Completing comprehensive mental health assessments.
• Completing Dialog+ assessments as part of required KPIs.
• Producing holistic risk assessments.
• Identifying and appropriately managing clinical and safeguarding risks.
• Managing an allocated clinical caseload independently.
• Participating in MDT meetings and morning safety huddles.
• Carrying out home visits where required.
• Liaising with GPs, families, carers and other healthcare professionals.
• Working with social care, housing, crisis services and voluntary-sector organisations.
• Signposting service users to appropriate services.
• Discussing complex cases with the multidisciplinary team.
• Supporting service users with recovery-focused interventions.
