About the Role

The City of Doncaster Council is recruiting two Social Workers to join its Integrated Discharge Team within the Home First Directorate.

The successful candidates will work in the Discharge Assessment and Review Team, based at the Civic Office in Doncaster. The team supports adults discharged from Doncaster Royal Infirmary, rehabilitation settings, the Positive Step social care assessment unit, and those returning home with support.

The service follows a “Why not Home, Why not today” approach, focusing on timely hospital discharge, independence, strengths-based assessments and positive outcomes for adults and their carers.

You will work closely with colleagues across health, social care, housing and voluntary services to identify suitable support, connect people with community resources and help reduce or delay the need for long-term care where possible.

Key Responsibilities

  • Complete assessments and reviews of care and support arrangements for adults following hospital discharge.
  • Support timely discharge planning using a person-centred, strengths-based approach.
  • Promote independence and help individuals achieve positive outcomes.
  • Identify risks and determine appropriate care and support needs.
  • Carry out duty functions and safeguarding enquiries.
  • Work with patients, families and informal carers to understand their needs and provide appropriate advice, referrals and signposting.
  • Apply relevant legislation and guidance, including the Care Act 2014 and Mental Capacity Act 2005.
  • Work collaboratively with health professionals and partner organisations to coordinate safe discharge plans.
  • Maintain accurate records and prepare clear assessment reports.
  • Manage several cases simultaneously in a fast-paced environment while meeting short deadlines.

Essential Experience and Skills

Applicants should have relevant Social Work experience, particularly in assessment and adult social care.

The council is looking for candidates with:

  • Knowledge and experience of assessments under the Care Act 2014.
  • Understanding of the Mental Capacity Act 2005 and hospital discharge policies.
  • Awareness of safeguarding responsibilities and GDPR.
  • Strong assessment, report-writing and recording skills.
  • Experience or knowledge of risk assessments and strengths-based practice.
  • The ability to manage competing priorities and work effectively under pressure.
  • Good communication and interpersonal skills when working with adults, families and professionals.
  • An understanding of carers’ needs and the support available to them.

The council welcomes applicants with transferable skills gained through other employment or voluntary work. Applicants should review the official job profile and demonstrate how they meet the required criteria in their application.

Working Pattern and Weekend Duties

The Integrated Discharge Team operates a seven-day service. Staff may occasionally work weekends at Doncaster Royal Infirmary, supporting discharge assessments and plans and providing cover for the emergency department.

Weekend working is currently approximately one weekend in every seven. The advertised posts include a 0.5% additional allowance, and two non-working days are incorporated into the work pattern to account for weekend duties.
