Job Overview

City Health Care Partnership CIC is recruiting an Urgent Care Practitioner to join the established multidisciplinary team at Goole Urgent Treatment Centre.

The successful candidate will provide timely assessment and treatment for patients presenting with minor injuries, minor illnesses and acute health concerns. The role involves working alongside experienced colleagues and partner organisations to ensure patients receive appropriate treatment, advice or onward referral.

There are also opportunities for ongoing learning, development and professional support.

Main Responsibilities

  • Assess, examine and manage patients with undiagnosed or acute health needs.
  • Diagnose and treat minor injuries and illnesses within your scope of practice.
  • Refer patients appropriately to support safe and effective ongoing care.
  • Support evidence-based practice, clinical audit and service improvement.
  • Provide guidance to junior colleagues and act as a positive role model.
  • Work effectively as part of a multidisciplinary team.
  • Maintain high standards of patient-centred care and professional practice.

Essential Requirements

Applicants should have:

  • A Degree in Nursing or Paramedic Science (Level 6), or significant relevant experience.
  • Relevant academic modules or specialist training.
  • Current UK professional registration with the NMC or HCPC.
  • Experience working in an acute or primary care setting.
  • Knowledge of unplanned care, relevant policies, legislation and guidance.
  • Evidence of continuing professional development and evidence-based practice.
  • Experience managing workloads and meeting deadlines.
  • Experience using software packages such as SystmOne and email.
  • Strong clinical assessment, decision-making and communication skills.

Previous experience with PGDs, multidisciplinary working, minor injuries and minor illness presentations, and the initial management and onward referral of patients with more serious or life-threatening conditions is desirable.
