Homeless Mental Health Practitioner – NHS
Greater Manchester Mental Health NHS Foundation Trust is recruiting a Homeless Mental Health Practitioner to join its Manchester Mental Health and Homeless Team.

Job Overview
This is a 12-month secondment for a full-time Band 6 Mental Health Practitioner position.
The role is open to qualified:
- Mental Health Nurses
- Social Workers
- Occupational Therapists
Candidates must have active registration with the relevant professional body.
The successful applicant will work with homeless individuals experiencing mental health difficulties, helping assess their needs and identify appropriate treatment pathways. You will work as part of an established specialist, multidisciplinary and trauma-informed homeless mental health team.
Main Responsibilities
The post holder will:
- Provide community-based engagement and mental health assessments.
- Support people experiencing complex mental health and substance misuse needs.
- Work alongside mental health and addiction services.
- Support rough sleepers through drop-in clinics at day centres and direct street outreach.
- Work in partnership with council staff, drug and alcohol workers, police and homeless outreach workers.
- Assess the mental health needs of homeless individuals who are not engaged with mainstream mental health services.
- Contribute to service planning and the future development of mental health support for homeless people in Manchester.
Salary and Contract
- Salary: £39,959 – £48,117 per year, pro rata

Requirements
Essential
- Qualification in a relevant professional discipline.
- Current UK professional registration.
- Community experience in mental health.
- Experience and understanding of mental health and/or dual diagnosis.
Desirable
- Experience within the homeless sector.
- Post-qualification experience working with complex mental health needs.
