Job Overview

About the Role

The successful candidate will work as a Senior Care Coordinator within the Learning Disability and/or Autism Spectrum Condition Keyworker Service covering Barnet, Enfield and Haringey.

The role focuses on coordinating the Dynamic Support Register (DSR) for children and young people with learning disabilities and/or autism who have mental health needs or are presenting with behaviours of concern.

Key Responsibilities

• Coordinate the scheduling of Care, Education and Treatment Reviews (CETRs).
• Coordinate and support LAEP meetings.
• Chair or support CETRs across Barnet, Enfield, Haringey, Camden and Islington.
• Support multi-agency approaches to care planning.
• Help children and young people remain safely supported within their communities.
• Review CETR and LAEP recommendations.
• Liaise with commissioners regarding emerging barriers and service requirements.
• Attend inpatient MDT meetings, CETRs and CPA meetings.
• Support discharge planning from CAMHS inpatient and residential settings.
• Work with Commissioners, CAMHS, Children’s Disability Teams and Social Care.
• Work with Adult Learning Disability and Mental Health teams.
• Maintain high standards of case recording.
• Use relevant information systems to record outcomes.
• Manage workload effectively and plan work in advance.
• Participate in supervision and DSR Team meetings.
• Work flexibly around the needs of children, young people and families.

Essential Professional Qualification

Applicants must have:

• Current registration as a Social Worker, Learning Disability Nurse or Registered Mental Health Nurse, or another relevant professional qualification.
• Evidence of continuing professional development.

Essential Skills

Candidates should demonstrate:

• Ability to coordinate regular and timely CETRs.
• Ability to support care planning that reduces avoidable admissions.
• Ability to work effectively with operational professionals.
• Experience delivering multidisciplinary and multi-agency care management.

Essential Experience

Applicants should have:

• A sound understanding of care management practice.
• Knowledge of commissioning and funding arrangements within statutory children’s health and care services.
• Experience overseeing complex support plans.
• Experience with risk assessment and assessment of need.
• Understanding of neurodevelopmental differences or disabilities.
• Understanding of behaviours of concern and mental health needs.

Desirable Experience

• Experience of commissioning within health or social care settings.
