Job Overview

The Homeless Health Inclusion Team at Imperial College Healthcare NHS Trust is looking for a motivated and compassionate professional to support patients experiencing homelessness who attend Accident & Emergency departments across the Trust.

The successful applicant will provide early intervention and practical support to help vulnerable patients transition safely back into the community while helping reduce repeated hospital attendances.

You will work independently within the Emergency Department while remaining closely connected to the wider Homeless Health Inclusion Team.

Main Duties of the Job

The successful candidate will:

• Work with Emergency Department colleagues and the hospital Homeless Health Team.
• Work with the local HIU ED Case Worker and Borough HIU Care Coordinators.
• Help implement referral and feedback mechanisms.
• Act as a central point of contact for referrals involving people experiencing homelessness.
• Provide immediate support to patients.
• Assist with housing, financial and wellbeing needs.
• Help patients access primary and specialist healthcare services following discharge.
• Build effective relationships with hospital and community partners.
• Work with GPs, ambulance services, police, mental health professionals, social services, community teams and third-sector organisations.
• Identify and respond appropriately to safeguarding concerns.
• Provide housing and homelessness advice to Emergency Department staff.
• Contribute to teaching and awareness sessions.
• Keep knowledge of housing legislation, welfare reforms and local services up to date.
• Identify personal learning and development needs.
