About the Role
This is a mental health-focused role developed as part of the Additional Roles Reimbursement Scheme (ARRS) model. The post holder will work within a defined Primary Care Network (PCN), alongside GPs and other primary care colleagues, while receiving clinical support from West London NHS Trust’s multidisciplinary mental health teams.
The successful candidate will provide mental health assessments and brief interventions for referred patients and help develop mental health expertise within a group of GP practices.
The role also involves supporting people with long-term mental health conditions, facilitating access to appropriate services and helping primary care teams manage patients whose care has been transferred back to their GP.
Regular supervision will be provided by a senior clinician within MINT, with the post holder working within an agreed scope of practice that is expected to develop as the service evolves.
Key Responsibilities
The successful candidate will be expected to:
- Provide mental health assessments, formulation and psychological interventions within primary care.
- Support appropriate access to MINT and other specialist services, including Early Intervention in Psychosis and psychological therapy services.
- Review and support GP practice lists of people with serious mental illness (SMI).
- Complete annual reviews for people with SMI in collaboration with GPs.
- Work proactively with GPs to identify people who may benefit from additional mental health support.
- Facilitate access to mental health services for people with long-term mental health conditions.
- Support transfers of care from West London NHS Trust MINT services back to primary care where appropriate.
- Screen and triage patients according to their mental health needs.
- Deliver appropriate short-term interventions.
- Support access to therapeutic activities.
- Work with voluntary, community and third-sector organisations.
- Provide advice, training and enhanced support to primary care clinicians.
- Help increase the confidence and skills of primary care teams in supporting people with mental health conditions.

Requirements / Person Specification
Applicants must have an appropriate professional qualification and current UK registration.
Essential qualifications include a recorded or registered qualification in a relevant profession such as:
- Psychology
- Mental health nursing
- Social work
- Occupational therapy
- Arts therapy
