Job Description:
Join Medimote as a Remote Care Management Coordinator and support Medicare patients with care management programs! You’ll engage patients, collect clinical information, and document interactions accurately.
Responsibilities:
- Conduct patient outreach and follow-ups for Medicare care management services
- Collect and document patient-reported symptoms, vitals, and care plan updates
- Communicate with patients in a compassionate and professional manner
- Support care coordination activities under clinical supervision
- Identify patient concerns and escalate to clinical team
- Accurately document encounters and communications
Requirements:
- CMA (Certified Medical Assistant) certification preferred
- Experience in chronic care management or clinical support roles preferred
- Strong verbal and written communication skills
- Ability to speak with patients compassionately and professionally
- Strong documentation and clinical note-writing skills
- Ability to work independently in a fast-paced remote environment
