About the Role
This is an excellent opportunity for an experienced Social Worker who is passionate about supporting adults with learning disabilities to live independently, safely, and with dignity.
Working within Somerset Council’s supportive Learning Disabilities Team, you will build meaningful relationships with adults, families, carers, and partner organisations while delivering person-centred and strengths-based social work practice.
The role focuses on empowering individuals to make informed choices, remain connected to their communities, and achieve positive long-term outcomes.
Key Responsibilities
- Complete strengths-based assessments for adults with learning disabilities.
- Assess risks and develop personalised care and support plans.
- Manage a varied caseload while ensuring regular reviews.
- Support individuals and families to make informed care decisions.
- Promote independence, inclusion, dignity, and choice.
- Respond appropriately to safeguarding concerns.
- Support individuals during crisis situations.
- Work collaboratively with NHS professionals, housing providers, voluntary organisations, and other agencies.
- Maintain accurate case records and documentation.
- Carry out carers’ assessments and support planning.
Essential Requirements
Applicants should have:
- A recognised Degree or Master’s in Social Work (or equivalent).
- Current registration with Social Work England.
- Experience or a strong interest in supporting adults with learning disabilities.
- Strong assessment, communication, and decision-making skills.
- Ability to work effectively within multidisciplinary teams.
- Knowledge of the Care Act 2014 and adult safeguarding legislation.
- Commitment to strengths-based and person-centred practice.
- Excellent organisational and time management skills.
- Full access to a vehicle for work purposes.
