Social Worker – Learning Disabilities (Adults)
Somerset Council is seeking an experienced Social Worker to join its Learning Disabilities Team in Yeovil. This role offers the opportunity to support adults with learning disabilities to live independently, safely, and with dignity while building meaningful relationships with individuals, families, and carers.
Working within a supportive multidisciplinary team, you will deliver person-centred care in line with the Care Act 2014 and help people achieve positive outcomes within their communities.

About the Role
As a Social Worker within the Learning Disabilities Team, you will support adults with learning disabilities and their carers by carrying out assessments, managing risk, developing personalised support plans, and promoting independence.
You will work closely with health professionals, housing providers, voluntary organisations, and other partner agencies to deliver coordinated, person-centred care.
Key Responsibilities
- Undertake strengths-based assessments
- Assess needs, risks, and eligibility under the Care Act 2014
- Manage a varied social work caseload
- Develop and review personalised care and support plans
- Support individuals, families, and carers
- Promote independence, dignity, and wellbeing
- Respond to safeguarding concerns
- Support adults during crisis situations
- Maintain accurate case records
- Work collaboratively with partner agencies
- Participate in multidisciplinary meetings
- Complete carers’ assessments and support planning
Essential Requirements
Applicants should have:
- A recognised Social Work qualification
- Current registration with Social Work England
- Experience or strong interest in Learning Disabilities services
- Strong assessment and risk management skills
- Excellent communication and interpersonal skills
- Commitment to person-centred, strengths-based practice
- Ability to work effectively with partner organisations
- Good organisational and case management skills
- Ability to prioritise competing workloads
- Fluent English in accordance with the Immigration Act 2016
- Access to a vehicle for work purposes
