About the Role
The Discharge Support Officer will work alongside Discharge and Transfer leads and ward teams to support the safe, effective and timely discharge or transfer of patients to the most appropriate care setting.
The role involves communicating with patients and their families, completing referrals to other services and working closely with healthcare professionals across the multidisciplinary team (MDT).
You will also help reduce avoidable delays in patient discharge and act as an important point of contact between ward teams, patients, families and external services.
Key Responsibilities
- Support the safe, effective and timely transfer of patient care from the Trust to the most appropriate setting.
- Take active steps to reduce the number of patients experiencing delayed transfers of care.
- Liaise with Community Care Coordinators, primary care teams, registered nurses, doctors, therapy services and Home to Assess teams.
- Work with the wider multidisciplinary team to support proactive and continuous discharge planning.
- Complete referrals to appropriate community and other services.
- Communicate effectively with patients and their families throughout the discharge process.
- Assist with training and supporting multidisciplinary staff in the use and updating of the Ward Interactive Whiteboard system.
- Coordinate activities associated with board round meetings.
- Act as the main point of liaison between ambulance providers and the Trust.
- Manage competing priorities and work to tight deadlines.
