This is a permanent Band 3 NHS role with the opportunity to complete a Level 3 apprenticeship qualification if you do not already hold the required Level 3 qualification or equivalent relevant experience.

About the Role
You will work as part of an integrated team supporting families, children and young people aged 0–19, and up to 25 for young people with SEND.
The role involves contributing to family health needs assessments and working with families and partner agencies to develop, implement and review individual and group packages of care.
You will also contribute to wider public health programmes for children and young people across individual, group and community settings.
Apprenticeship Opportunity
Applicants who do not already have a relevant Level 3 qualification or equivalent experience may have the opportunity to complete a relevant Level 3 apprenticeship alongside the role.
The apprenticeship provides:

Key Responsibilities
You will:
- Support family health needs assessments.
- Work with children, young people and their families.
- Develop and review packages of care.
- Work with other agencies and professionals.
- Contribute to individual, group and community programmes.
- Support families and patients who may be distressed.
- Maintain confidentiality and handle sensitive situations appropriately.
- Participate in safeguarding, management and clinical supervision.
- Work effectively both independently and as part of a wider team.
Essential Requirements
Applicants should have:
- English and Maths at Level 2 or equivalent.
- NVQ Level 2 in a relevant subject.
- NVQ Level 3 in a relevant subject or willingness to undertake a relevant Level 3 apprenticeship.
- A genuine interest in caring for others.
- Good interpersonal and communication skills.
- Basic IT skills.
- The ability to work independently within agreed boundaries.
- The ability to work effectively with teams and groups.
- The ability to support and reassure patients or carers in distress.
- An understanding of confidentiality and sensitivity when dealing with difficult situations.
Experience in early years education, community settings, patient care or team-based work is desirable.
