Job Details
Shropshire Council is recruiting a Homeless Prevention Support Worker to support care leavers and previously looked-after young people who are homeless or at risk of homelessness.
The successful candidate will work with a small caseload, helping young people secure suitable accommodation, maintain their existing housing and address issues that could put their accommodation at risk.
This is a temporary 24-month position, working 37 hours per week. The contract may be extended for a further 12 months.
About the Role
The Homeless Prevention Support Worker will place the Care Leaver at the centre of assessment, planning and support.
You will build positive working relationships with young people and work alongside Personal Advisers, social workers, housing colleagues and other agencies to help prevent homelessness and improve housing outcomes.
The role will involve supporting young people with issues such as debt, behaviour, substance use and mental health where these factors are affecting their housing situation.
Key Responsibilities
- Manage and co-work a small caseload of care leavers and previously looked-after young people.
- Complete needs assessments and identify appropriate housing-related support.
- Develop individual housing support plans.
- Review support plans following progress, changes in circumstances or significant events.
- Help young people who are homeless or at risk of homelessness find suitable accommodation.
- Support young people to maintain their existing accommodation where possible.
- Help address issues including debt, behaviour and substance use that may affect housing stability.
- Work closely with Personal Advisers and colleagues within the housing service.
- Liaise with social workers, housing professionals, probation, police and other partner agencies.
- Advocate for Care Leavers when requested to help ensure their needs are appropriately addressed.
- Maintain accurate records and relevant databases.
- Produce quarterly reports relating to the service.
- Attend homeless prevention meetings with colleagues from other authorities involved in the government-funded programme.
- Contribute to efforts to reduce homelessness among the care leaver population.
