About the Role

Greater Manchester Mental Health NHS Foundation Trust is seeking a highly motivated and experienced Duty Worker to join the North Trafford Community Mental Health Team (CMHT).

The successful candidate will work within an integrated health and social care community mental health service, supporting vulnerable adults experiencing mental health difficulties.

The role involves screening and triaging referrals, completing comprehensive assessments, managing risk, contributing to multidisciplinary team discussions and supporting appropriate care planning.

The service works with people aged approximately 16–70 years across Trafford who are experiencing mental health difficulties.

• Mental Health Occupational Therapist

Current UK professional registration is required.

Main Duties and Responsibilities

The successful candidate will be responsible for:

• Carrying out initial assessments for new referrals received by the team.
• Screening and triaging referrals.
• Redirecting unsuitable referrals to appropriate services.
• Completing robust and holistic risk assessments.
• Assessing the needs of vulnerable adults with complex care requirements.
• Conducting evidence-based biopsychosocial assessments.
• Developing effective professional relationships with patients and partner agencies.
• Liaising with multiple agencies and third parties.
• Making onward referrals where appropriate.
• Advocating for vulnerable adults to help achieve positive treatment outcomes.
• Preparing and completing reports within appropriate timescales.
• Contributing to weekly multidisciplinary team (MDT) discussions.
• Supporting care planning and treatment decisions.
• Working collaboratively with colleagues across health and social care services.

Essential Qualifications

Applicants must have one of the following:

• Professional registration as a Mental Health Nurse

Desirable Qualification

• Post-registration qualification in a clinically orientated area.

Essential Experience

Candidates should have:

• Post-registration professional experience.
• Recent experience working in adult mental health or CAMHS.
• Experience completing biopsychosocial assessments.
• Experience assessing vulnerable adults with complex care needs in a community setting.

Desirable Experience

• Experience of mental health issues.
• Experience in a caring role involving someone with mental health needs.

Essential Knowledge and Skills

• Experience completing comprehensive biopsychosocial assessments.

• Ability to undertake effective risk assessments and risk management.
