About the Role
The successful candidate will work within Sefton’s Adult Social Care service, supporting adults with learning disabilities and/or autism through high-quality, strengths-based assessments, care planning and reviews.
The role focuses on promoting:

You will work with individuals, families, carers, health professionals and partner agencies in accordance with the Care Act and associated legislation.
Key Responsibilities
Strengths-Based Assessments
Undertake Care Act assessments, reviews and support planning, ensuring individual outcomes, independence and wellbeing remain at the centre of interventions.
Caseload Management
Manage a varied caseload, including complex cases, safeguarding concerns and transitions into adulthood.
Multi-Disciplinary Working
Work collaboratively with health colleagues and other professionals to deliver integrated support and achieve positive outcomes.
Joint Funding
Support individuals and families in navigating services, including Continuing Healthcare (CHC) and Section 117 arrangements where required.
Mental Capacity Act
Apply the Mental Capacity Act (MCA) in practice, including capacity assessments, best-interests decision-making and promoting least-restrictive options.
Safeguarding and Risk Management
Contribute to safeguarding and risk-management processes, ensuring proportionate and defensible decision-making.
Professional Development
Participate in team discussions, supervision and service development while sharing knowledge and supporting colleagues.
Candidate Requirements
The successful applicant should demonstrate compassion, professionalism and creativity, together with a strong understanding of Adult Social Care.
You should have:
- A Social Work degree or equivalent qualification
- Registration with Social Work England
- Experience in Adult Social Care
- Strong knowledge of the Care Act and related legislation
- Confidence managing complex, person-centred decisions
- Excellent partnership-working skills
- Experience working with health and community stakeholders
- Commitment to equality, diversity and inclusive practice
- A passion for continuous learning and professional development

The successful Social Worker will receive support from:
- 2 dedicated Lead Practitioners
- A dedicated Team Manager
- A Service Manager
- An experienced and inclusive leadership team
