About the Role
The successful candidate will become part of the specialist Dementia Team, working closely with healthcare professionals to provide advanced clinical assessment, diagnosis, treatment planning, crisis intervention, and ongoing support for individuals living with dementia.
The role focuses on improving patient outcomes by ensuring timely diagnosis, preventing avoidable hospital admissions, supporting early intervention, and facilitating safe discharge from hospital where appropriate.
This position combines advanced clinical practice, leadership, education, and collaborative working across community and mental health services.
Key Responsibilities
- Conduct comprehensive clinical assessments for patients living with dementia.
- Diagnose, assess, and manage complex patient needs.
- Develop personalised care plans alongside patients, carers, and families.
- Deliver evidence-based treatment and follow-up care.
- Prevent unnecessary hospital admissions through early intervention.
- Provide crisis assessment and rapid response where required.
- Support safe and timely discharge planning.
- Offer clinical leadership within the multidisciplinary team.
- Collaborate with primary care, mental health services, community services, and social care providers.
- Maintain accurate clinical documentation and patient records.
- Promote best practice in dementia care across the service.

Essential Requirements

Training Opportunity
This vacancy is open to:
- Qualified Advanced Nurse Practitioners.
- Existing NHS Band 7 Nurses.
- Experienced NHS Band 6 Nurses wishing to progress into Advanced Practice.
Applicants who do not yet hold a Master’s qualification may be appointed under Annex 21, allowing them to undertake fully supported Master’s level Advanced Practice training.
Following successful completion of the programme, trainees will progress into a substantive Band 7 Advanced Nurse Practitioner role.
Salary
- Band 7 Salary: £52,845 – £61,466 per year.
Trainees appointed under Annex 21 will receive a proportion of the Band 7 salary while completing their approved training programme.
